SAFETY AT SEA COURSES

Basic Safety Training (BST)
|
1. Three (3) pcs. 1 x 1 latest ID picture
2. Photocopy of Birth Certificate (NSO authenticated)
3. Transcript of Records or Diploma or School Certification
4. Four (4) pcs. 2 x 2 color picture on a white background, with name tag and in uniform (for marine graduate) or plain white polo (for non-marine)
5. Medical certificate-valid within 3 months of the intended training period
6. Accident Insurance Policy (AIP)-valid within the intended training period
BST enrollees with shipboard experience are required to submit a photocopy of Certificate of Service or SIRB (first and last page only)

Proficiency in Survival Craft and Rescue Boats other than Fast Rescue Boats
|
1. Photocopy of BST certificate (no shipboard experience) or photocopy of Certificate of Service or SIRB (enrollees with shipboard experience)
2. Medical Certificate (same as BST)
3. Accident Insurance Policy (AIP)
4. One (1) pc. 2 x 2 color picture on a white background, with name tag and in uniform (for marine graduate) or plain white polo (for non-marine)

Advance Training in Fire Fighting
|
1. Photocopy of BST certificate (no shipboard experience) or photocopy of Certificate of Service or SIRB (enrollees with shipboard experience)
2. Medical Certificate (same as BST) and ECG tracing with reading
3. Accident Insurance Policy (AIP)
4. One (1) pc. 2 x 2 color picture on a white background, with name tag and in uniform (for marine graduate) or plain white polo (for non-marine)

For Upgrading Courses
|
1. Three (3) pcs. 1 x 1 latest ID picture
2. Photocopies of the following documents:
a. Latest PRC license
b. Radar Nav certificate for RSC
c. Valid RSC Certificate for Radar ARPA
d. SBFFT for GTF
3. PRC license for Deck Officers and 1st class RTG Operator's Licence for Radio Officers taking up GOC for GMDSS
4. One (1) pc. 2 x 2 color picture for each course (with name tag, in uniform on a white background)
5. Pencil, compass-divider, pair of triangles (for RS and Radar Nav/ARPA enrollees)
6. Referral letter for OWWA scholars
7. Guarantee letter for company-sponsored trainees
